The nightmare of finding a bra that fits
bra measurementPlastic Surgeons usually consider 1 cup size to be about 150 mls or cc. This means that when deciding on an implant size a 2 cup size increase is about 300 cc. When the implant is under the muscle you need to allow about 75-100 cc more. However none of this will help you when trying to decide what size of bra you need now or after surgery. There are two important measurements to make. These measurements are in inches and apply mainly to the North American market.
The first is the band size around the chest. Use a tape-measure around the chest just under the breast. Keep the tape level on the front and back. Add 5" to this measurement to obtain your chest band size. 
The second is the cup size. You should measure around the breast at the fullest part. The difference between this measurement and your chest band size is your cup size. Each inch of difference is a cup size.

Deciding about the best incision, and scar, to use for breast augmentation
incision for breast augmentationPatients are often confused by the range of incisions that are used for breast augmentation. The four most common are: under the breast (inframammary), around the nipple (periareolar), under the arm (axillary) and in the navel (umbilical).
None of these incision is perfect. Each has good and bad features. It is not always possible to get the incision exactly where it is planned. Most scars are about the same length, 5 cms (2 inches), because this is the length needed to make the pocket and put the implant in good position.
The most widely used incision is on the undersurface of the breast. This has the advantage that it is concealed in shadow. The surgeon often tries to place the incision not in the crease but just above, so that when the arms are raised above the head the scar does not drop below a bikini or bra line. This also has the advantage that if you want to wear an underwire bra the underwire is less likely to press on the scar and be uncomfortable. This incision gives good visibility for the surgeon and makes placement of the implant more accurate. It will always leave a scar on the surface of the breast. This incision is used for placing the implant both above and below the pectoral muscle.
Another common incision is around the nipple. The scar is placed at the edge of the areola (dark) part of the breast and the normal skin. If it heals well it is often inconspicuous but if it heals poorly it may change the shape of the nipple. It may also reduce sensation in the nipple and is not usually recommended for women who may want children and breast feed. This incision is also used for placing the implant both above and below the pectoral muscle.
The armpit incision is usually used for placement of the implant below the muscle although it can be used to place above the muscle. It has the advantage that if it heals well it is inconspicuous. However it has three disadvantages that make it less possible today than a few years ago. These are first it is impossible to hide so that a patient in a swim suit who raises her arms above her head will show the scar. Second, if it heals poorly it may make shaving the armpit difficult in some people. Finally, because the distance from the incision to the a common site of bleeding (the inner lower part of the breast), some patients require a second incision on the undersurface of the breast. If complications occur they are often more difficult to treat from this approach.
The incision around the navel has not gained wide popularity but may suit some patients. It is concealed in the navel which makes the scar difficult to see. However technically the surgeon must tunnel from the navel and does not visualize the pocket unless endoscopic methods are used. The implant may also be damaged in passing it into its position on the chest. This method is often used to place empty saline filled implants that are filled when in position. As gel implants return to more general use this method may be less popular. This incision is used for placing the implant both below and above the pectoral muscle, but may be easier above the muscle.

Breast enhancements more popular with college women
Oops, someone's done it again. In recent years, the number of Britney Spears fans has shriveled, but the controversy surrounding her chest remains. While Spears continues to maintain that her assets are real, there are a rising number of college women who don't. In 2001, American women spent $668.9 million on breast augmentation, according to the American Society of Plastic Surgeons. Women between the ages of 19 and 34 accounted for 56 percent of the money spent. The number of women in this same age group who sought breast augmentation increased from about 19,500 in 1992 to more than 108,000 in 2000. Despite a decade of silicone scares and other controversies, breast enhancements are becoming more popular, especially among college-aged women. SMU women are no exception, spending an average of $3,043 per rack.

Is surgery better than a resume?
Some people turn to career coaches or resume doctors for help finding jobs. Others join networking groups to locate new clients. And then there are those who seek out plastic surgeons to give them an edge. Brian Bretan wants no one to think that he's run out of steam. The 59-year-old job seeker from Smithtown, N.Y., has had doctors remove the bags under his eyes, give him a fuller head of hair and erase his age spots. Now, he said, "I like the way I look." He's had two job interviews for pharmaceutical sales positions in the past couple of weeks, and expects to have at least two more in the coming weeks. His confidence is high given that these are his first interviews in more than 30 years. "I haven't hit any disasters yet," he said.

Tests to see if a silicone gel breast implant is broken
x-rayThe only certain way to know if a silicone gel breast implant is broken is to do an operation and look. This is not acceptable to many patients so other tests are used. These can help the patient and the surgeon decide about the need for surgery.
The most reliable test is an MRI scan which has a greater than 95% accuracy of predicting rupture. Unfortunately this is expensive and is used in circumstances where other tests are less reliable or a decision is essential.
Mammograms are not very reliable unless there is leak outside the capsule surrounding the implant.
Ultrasound is a difficult technique to perform but in the hands of an expert can show rupture inside the capsule in about 60-70% of patients.
There are no reliable blood tests to detect rupture of a gel implant. Examination of the patient may suggest a rupture if there has been any change in the size, firmness or shape of the breast.
It is not clear if an older implant is more likely to break or leak but if it is more than ten years old or has been subjected to a closed capsulotomy you should discuss this with your plastic surgeon.

Male ritual circumcision
New BMA guidelines about the ethics of male circumcision are published today. Dr Michael Wilks, Chairman of the BMA Ethics Committee, said today: "We hope these guidelines help doctors in their discussions with patients who ask for their sons to be circumcised."
The following factors can help doctors assess a child's best interests in relation to non-therapeutic circumcision:
* the patient's physical and emotional needs;
* the risk of harm or suffering for the patient;
* the views of the child if he is old enough to participate in the decision;
* the views of parents and family;
* the implications of performing, and not-performing, the procedure;
* relevant information about the patient's religious or cultural background

SWELLING: this can cause separation of the skin edges or even loss of skin (slough). This is sometimes due to the wound being closed too tightly.Late swelling, called edema, can be due to either a tight closure or more seriously damage to the venous or lymphatic drainage from the area.
BRUISING: the skin may be undermined and this can leave a space for fluid or blood to collect (see below).
BLEEDING: early on bleeding may indicate leakage from a blood vessel and require surgery. Slow bleeding may collect under the skin forming a "hematoma". Sometimes serum may also collect forming a "seroma".
INFECTION: this is uncommon but because of the closeness of the axilla the germs in this area can contaminate the wound.
SCAR: this is the most common problem following a brachioplasty. If the scar is placed on the front or back surface of the arm it is more visible. It is best if located on the lower surface of the extended arm. In some patients the scar may become thick or hypertrophic.
NUMBNESS OR CHANGE IN FEELING: damage to superficial nerves in the fat of the upper arm may cause numbness or feeling change, including painful sensations.
CONTOUR DEFORMITIES: in the central area of the incision these may be bands, folds or constrictions. Ateach end of the wound "dog ears" may form.
PATIENT DISSATISFACTION: as with all surgical procedures the patient may not like the final result.

Bleaching Products Influence Skin Disease Presentation
Over half of women presenting to a dermatology clinic in Senegal have used bleaching products cosmetically, influencing the presentation of skin diseases. Researchers from Institut d'Hygiene Sociale, Dakar, and other centres in Senegal and France asked a representative sample attending the clinic about their use of cosmetic bleaching products. Of the 368 women interviewed, 52.7% admitted currently using them.

Brazilian Firm Pumps Up Silicone Breast Business
Exuberant breasts exposed by beauty queens at Rio de Janeiro's Carnival or those stretching scanty bikini tops on Brazil's beaches often have little to do with nature's generosity. In fact, a small plant on the ugly outskirts of Rio de Janeiro that produces transparent silicone implants stands behind most of the so-called turbo-charged cleavages in the tropical country famous for its cult of body beautiful. Apart from endowing with silicone eight out of every ten Brazilian (news - web sites) women who choose breast enlargement surgery, Silimed, as the Brazilian manufacturer is called, exports 65 percent of its output and is the world's No. 3 supplier of breast implants. This summer, Silimed will open a new plant capable of producing 500,000 silicone packs annually, more than three times the size of its current capacity of 150,000 pieces, as it prepares an assault on the U.S. market -- the world's biggest. Using certified raw materials imported from the United States, Silimed mostly makes breast implants but also facial, buttocks and even testicle implants of silicone gel. "We are going to make a real go on the U.S. market in 2004 or 2005 ... We can take 10 percent of their market easily from the very start," said Silimed President Antoine Robert, a Frenchman who has lived in Brazil since 1974.

Genital cosmetic surgery introduced to Canada
New type of cosmetic surgery offered in Calgary.This story is about a cosmetic procedure that has just been introduced in Calgary. It’s a procedure performed on a woman's genitals. Dr. Bruce Allan is an Obstetrician and Gynaecologist. He agreed to let reporter Erika Taylor get a first hand look at this new cosmetic surgery for women. Allan is the only qualified surgeon in Canada to perform this kind of surgery. It's called Lazer Labia Plastie - the first of it's kind ever done in Canada. Over the last 5 years Dr. Allan has been training in California to bring this technique here. “I really had never though that much of doing this type of surgery until I found there was a demand out there that there were women who wanted the procedure done,” said Allan. ‘Amy’ will be the first woman to have this surgery performed in Canada. Dr. Allan will about trim 4.5 centimetres of skin.
Amy's surgery is to reduce the size of her vaginal lips. She finds there are not aesthetically appealing and they bother her somewhat when she wears tight jeans or rides a bike. “It’s just something that's been bothering me for quite a while since I was younger and it's something that just got in the way whether I’m urinating or having sexual intercourse,” said Amy, which is not her real name. - copyright Bell Globemedia -

Neonatal procedure suspended.Hunter Health has ceased routine neonatal circumcision within its hospitals following a Royal Australasian College of Physicians recommendation.In its position statement on circumcision, the college stated that it had revised its policy following 'critical literature analysis' by a working party of representatives from a number of professional organisations including paediatricians, paediatric surgeons, general surgeons and urologists from Australia and New Zealand.The Royal Australasian College of Physicians' division of paediatrics and child health has concluded that where no medical indication for circumcision exists, the procedure is best performed under general anaesthetic after the age of six months to reduce the risks and the discomfort for the child.

Slimming fad 'starts in playground'
About 20% of children have been teased about being fat. Girls as young as nine are resorting to dieting after being teased about their weight, say British researchers. One in five nine-year-old girls and nearly as many boys have been bullied by classmates for being fat, a study has found. Experts are concerned the taunts are contributing to the slimming fad among children, particularly young girls. One in five girls are dieting by the age of nine, putting them at higher risk of developing an eating disorder in later life.

earSometimes it is hard to know if you really have a problem. The ear is an easily visible organ so here is a guide so you can make a diagnosis and see if surgery will help you. To understand this information you will need to refer to our page on ear anatomy which you may want to print for reference.
For most patients the protruding ear is obvious. In young children the parent or other family member may notice this first. Even young children may be embarrassed by remarks made by school friends. Sometimes it may run in a family and a parent or sibling may have the same problem.
Although the ear may be larger than normal it is more common for it to be a normal size. It most commonly affects both ears.
The problem is usually due to an underdeveloped antihelical fold or a overdeveloped or deep concha. When both factors are present the ear will stick out even more.
The normal ear is between 1.5 and 2.0 cm ( 1/2 to 3/4 of an inch) from the scalp to the outer part of the upper ear. If it is more than this it is abnormal. The outer edge of the ear should be smooth and the angle between the concha and the rest pf the ear about 90 degrees. When you look at the ear from in front you should be able to see the helix behind the antihelix. In some patients the earlobe may be larger.

It's the time of year when accountants transform into superheroes, swooping down to save us from our tax-code illiteracy and shameless demands for deductions on our electric toothbrush ("We all need good breath when dealing with clients."), and our four plane tickets to the Bahamas ("I need to be abreast of the competition in my job as a Statue of Liberty tour guide."). At the same time, they warn that we might actually miss valuable opportunities. Used jock straps, tummy tucks and gambling debts may raise a few eyebrows at the Internal Revenue Service, but they can be as legitimate as a home mortgage when it comes to personal tax filing.

Plastic surgeons under-utilised
Despite the availability of adequate number of plastic surgeons, their services are not properly utilised in the health sector, according to Dr K Sridhar, president of the Association of Plastic Surgeons of India.Though the plastic surgeons were “refined surgeons”, the public had so far failed to recognise them and had not been approaching them directly, to set right their defect, deformity and disability, Sridhar told reporters here today.Claiming that plastic surgery was in practice even before plastic was invented, Sridhar said the government should integrate resources by joining hands with the private sector to popularise this system.

Hamsters immunity damaged by liposuction
Sudden Removal Of Fat Impairs Immune Function In Rodents, Biologists Find. A liposuction-like procedure called lipectomy results in a loss of humoral immune protection in two commonly studied rodent models, the prairie vole and the Siberian hamster, scientists have found. The report by a team of researchers at Indiana University, Ohio State University and Johns Hopkins University was made available online this week by The Royal Society. Their study is the first to show that even a moderate loss of fat leads to decreased amounts of infection-fighting IgG antibodies. "We were able to show that even a subtle decrease in fat can decrease humoral immunity, which has the potential to increase disease susceptibility," said Indiana University biologist Gregory Demas, who led the study. "We knew that immune function is energetically costly, but it is now clear that animals use energy stored as fat to bolster immunity and likely to combat infection." The researchers also found that immune system function improved after the regrowth of fat tissue that had been removed.

Latest surgery craze sounds like a tall tale.The newest trend in cosmetic surgery for Chinese women is both painful and popular, and takes the quest for beauty to new heights. For centuries Chinese women bound their feet tightly in the name of beauty. That practice has long been banned, but it's been replaced by a new trend --having their legs lengthened. As many as 200 women a year sign up for the surgery at cosmetic clinics, having their legs cut and lengthened with metal rods. All so they can be taller. "Everyone has their own ideal of beauty, and I can't reach mine with out his surgery," says Zhang Ting, who is just minutes away from the operating table. She has saved up for the price of the surgery, $15,000, by teaching cosmetics and beauty classes. "It's definitely worth all the money and the pain," says Wang Yun, a 23-year-old student who had the surgery five months ago. She's 10 centimetres taller, but not yet able to walk. According to the Beijing Institute of External Skeletal Fixation Technology, there is an increasing demand for the procedure. Patients can expect an initial two-week stay in hospital, and several months of recovery. Those who are determined to achieve great heights can have the surgery done twice, with a two-year wait in between. Most clients are women, but a few men have also had their legs lengthened.

That inescapable problem called parents. We take them for granted when we need them, then we outgrow them. What then? Eighty-six percent of grown children surveyed by DaCapo claim to have a good relationship with their elderly parents. Is it really that simple? Of course not. Rina Igarashi (a pseudonym) is 19, and, filial daughter that she is, hesitated to undergo longed-for cosmetic surgery on her eyelids. True, her eyes were narrow and Oriental; on the other hand, her face is her inheritance from her parents. Didn't she owe it to them to respect it? She talked her parents into sanctioning the surgery, which, no sooner done, put new ideas into her head: She wanted her nose raised, her breasts enlarged. "Absolutely not!" cried her parents. "Never mind," she thought, "I'll do it when I leave home. But," she vows, "I'll make it up to my parents -- I'll buy my mother a face lift!"

Texas may owe Medicaid $300 million, State officials say they shouldn't be held liable for overpayments made with federal approval.The state could be on the hook for returning more than $300 million to the federal government for misspending Medicaid money.The challenge of coming up with that money comes as lawmakers are already looking at massive cuts to the public health system to help fill the state's $9.9 billion budget hole.
In one case, the program reimbursed a hospital for performing elective plastic surgery on at least 13 patients.

How does a laser work on human tissue?
operating roomThe laser is a highly focussed beam of light . When it reacts with human tissue it produces intense heat. There is nothing magical about what the laser does but many patients are mislead by reports of almost magical effects. When reading such reports it helps to remember that the laser is acting to produce heat and burn the tissue. Although this is in a very focussed way.
The doctor must direct the beam of laser light to exactly the right place. If it hits tissue which was not planned, serious damage can occur. As with all resurfacing procedures complications can occur using the laser.
When the laser beam reaches the tissue its energy is absorbed by the cell. This causes the cell to rapidly heat up. Water is lost and the cells destroyed.
The amount of heat produced depends on how much energy is absorbed. This is related to the amount of water in the cell, the cell's pigment type and the wavelength of the laser (the type of laser).

Ingredients in hundreds of popular anti-ageing and beauty treatments could actually be causing wrinkles, according to researchers. They have found that acids used in some skin care products increase sensitivity to sunburn, which can lead to ageing. The finding may bring about health warnings on creams and lotions. At the centre of the concerns are AHAs, or alpha hydroxy acids. They are sometimes called fruit acids and have become a staple ingredient in beauty creams. Essentially, the chemicals help peel away old skin. But this can expose the new and more sensitive skin layer to damage by the ultraviolet radiation in sunlight. AHAs may be listed under a host of names, including lactic acid, triple fruit acid, sugar cane extract and glycolic acid. Draft guidance from America's Food and Drug Administration suggests that a warning should appear on the labels.

Feats of clay, Luke Shepherd uses sculpture to show people who are about to have plastic surgery what they will look like afterwards. “Fat cheeks might be cute in baby photographs,” says Katie, 34, “but by this point in my life I’m pretty sick of looking in the mirror and seeing a hamster.” With her mix of Native American and Asian blood, her almond-shaped eyes and full mouth, Katie is definitely a head-turner. But despite this, she has been considering having plastic surgery to remove some of the fat from her cheeks to heighten her cheekbones and to reshape her face. “The only thing that’s been stopping me is the fear of the unknown, the worry that I’ll turn out lopsided and end up looking worse than I do now,” she says. “That’s the No 1 fear for every woman considering cosmetic surgery.” Recently, however, through a friend in the medical world, she heard about Luke Shepherd, a sculptor who may have found a way to allay her pre-operative fears — by showing in clay just how she might look when the plastic surgeon’s work is done.

But let's get one thing straight: Yes, these surgeons perform all the cosmetic procedures associated with the field — tummy tucks and nose jobs, face lifts and botox injections, spider vein removal and liposuction. What really gets them up in the morning, though, is the other half of the business: reconstructive surgery for those recovering from illness, injury or birth defects. "Plastic surgery is trying to make people whole again," says Dr. John Fagg, one of the four practice principals." The surgeons pride themselves on being able to perform the full gamut of plastic procedures, helping women enlarge their breasts, on the one hand, or repairing a child's cleft lip on the other. They are not a fat-sucking factory, nor a spa center that happens to offer eyelid sculpting, though they do a lot of that.

SMOOTH OUT SURGERY INSURANCE PROBLEMS. If you are thinking about having plastic surgery, you're not alone. Around 75,000 people willingly go under the knife each year.
Whether you want plump lips, smaller hips or a smoother forehead, it's only a treatment away - provided you have the cash. But what happens if something goes horribly wrong? Cosmetic surgeons are required by law to have medical indemnity cover, which pays out should a patient successfully claim. In recent years, British patients have won £7million in compensation, according to the Medical Defence Union (MDU), which provides legal advice and indemnity insurance to doctors. ''The most common reason for bringing a claim is dissatisfaction with the aesthetic result of the procedure, for example, patients who thought surgery would enhance their looks but were disappointed with the results,'' says Dr Karen Dalby, clinical risk manager at the MDU.
