bleeding after chin implantation (augmentation)

Bleeding after chin implantation can lead to a hematoma or blood clot in the pocket around the implant. This may have serious results if it becomes infected. Infection may be more likely if the dissection was through the mouth because of the bacterial contamination in this area.

A hematoma is more likely if the dissection was difficult and traumatic. It may also occur if the patient's blood pressure is raised in the first few hours after surgery. This is often because of vomiting. In some cases bleeding is due to an injured after surgery.

Unfortunately unless the hematoma is large it is difficult to assess immediately after surgery because of swelling. A hematoma may also mimic asymmetry , making it difficult to know if the implant is well positioned

Fortunately most hematomas resolve without treatment. They may lead to infection or excess fibrous scarring around the implant and defects in contour. If the pressure is marked the position of the implant may be changed.

If the hematoma is large, surgical treatment is usually need. This involves opening the pocket and removing the blood clot. The pocket is then washed out and the implant reinserted. A drain is occasionally used.

At the time of surgery the surgeon will try to avoid this problem by making certain that all bleeding has stopped before closing the wound. Many surgeons feel it may help to make the pocket the same size as the implant and avoid unnecessary dissection. When the wound is closed the area between the skin and the implant should be closed with layers so that there is no space for a hematoma to form.

Although tapes and bandages may be used many surgeons feel that they probably make little difference to the likelihood of bleeding and hematoma formation.
